My Health Team (MyHT)
In December 2019, Brandon Clinic Medical Corporation partnered with My Health Team Brandon. The Brandon Clinic My Health Team supports patients who call Brandon Clinic their “home clinic”
What is a home clinic?
Home Clinics are primary care clinics that act as your home base within the health care system. Home Clinics provide patients with timely access to care, coordinate their health care within the health care system, and manage their health care records. Having a home base helps support comprehensive and continuous care throughout a patient's life.
What are My Health Teams (MyHTs)?
My Health Teams are partnerships that bring different care providers together (whether located in the same office or connected online) to work with you to make sure you get the care you need when you need it. MyHT includes your doctor or nurse practitioner but also other health professionals such as a social worker, dietitian, pharmacist and mental health shared care counselor. As a patient of My Health Team, you will be treated as a partner in planning and managing your care and will be provided with the tools and resources to help you do so.
The Brandon Clinic’s MyHT include a Registered Dietitian, Clinical Pharmacist, Mental Health Worker, Service Navigator support and access to Cognitive Behavior Therapy Classes.
Other services that are available to Brandon Clinic patients are Clinical Psychologist, Physiotherapy and Financial Facilitators.
How to access My Health Team Providers?
Brandon Clinic patients can be referred to these providers through their family doctor and in some cases, can self-refer for their services.
Registered Dietitian
MyHt Registered Dietitian provides enhanced chronic disease self-management for clients. MyHT RDs work to improve client outcomes and maintain health status as well as to reduce avoidable emergency room visits and hospital admissions.

Clinical Pharmacist
MyHT Clinical Pharmacist provides medication management for clients with chronic and complex health conditions where medication adherence and reduction in adverse medication events can improve and maintain health status as well as reduce avoidable medication-related emergency room visits and hospital admissions.
Pharmacist Priorities

Complex medication regimes (i.e. 10-15 medications, high alert medications)

Medication reconciliation due to recent hospital discharge or specialist visit.

Clients with 2 or more chronic conditions (i.e. CHF, Hypertension, Diabetes, COPD, Asthma) OR one chronic condition requiring specialized care (i.e. client with IV)

Practitioner referral (may include, but not limited to; controlled medication use, chronic pain, Pharmacare)

Smoking Cessation Counselling
Pharmacist Access
Completed referral form by primary care physician or nurse practitioner.
Clients can also self-refer to this service.

Mental Health Worker
The MyHT Community Mental Health Workers - Shared Care provide brief treatment (6 - 8 sessions) for patients experiencing anxiety, depression or adjustment disorders. They can be co-located in primary care clinic(s) to increase collaboration between providers, share in client care, reduce stigma and increase access to mental health services. They maintain a strong connection to other RHA Community Mental Health programs and work collaboratively with the care team in the clinic(s) and community services to improve and maintain client’s mental health status as well as reduce mental health related emergency room visits and hospital admissions.
Mental Health Worker Priorities

Clients with anxiety, depression or adjustment disorder(s) who’s care plan would fit a brief treatment model (6-8 sessions)
Mental Health Worker Access
Patients are referred by their primary care physician or nurse practitioner.
Respecting privacy and consent requirements, client information can be shared with primary care providers and other community programs/services, as needed
